Hi Larry its the Euro-Vox Max Digital cable receiver there is no model number, I have had the receiver for 10 months with no problems at all apart from some re-tuning issues.
If the receiver is not under a warranty, it is hard to get parts or a service manual for this unit. I am sorry to say, it is going to be hard to get parts and or a service manual for this unit. I would first trh to get it repaied or replaced under warranty.
